Medical Withdrawal Request Procedure
- Complete an application for accommodations by visiting the department’s webpage. The department’s webpage may be found on the ºÚÁÏÌìÌÃ’s website.
- Read and review the full Medical Withdrawal Policy.
- A personal letter from the student that details the reason for the medical withdrawal request
- A letter from a licensed professional that details the need for the medical withdrawal for the specific term.
- Upload supporting documentation with the application OR email / fax the documentation to the email address or fax number below
- Check your JagMail account for a follow-up email. It may take 2 weeks to receive a follow up message.
- After receiving a message via JagMail, schedule an accessibility via Navigate ºÚÁÏÌìÌà OR visit staff during virtual office hours on Zoom. The dates, times, and Zoom links for virtual hours are listed on the homepage for the department.
- After meeting with departmental staff, during the accessibility meeting, you will receive more information regarding the next steps of the medical withdrawal request.
